Student-athletes, listen up, it’s that time again when you need to get a sport’s physical for the upcoming season.
This Saturday, Tomball High School is partnering with Houston Methodist to make this possible. They will be here from 9 a.m. to 1 p.m.
You must have an appointment, which can be purchased online; physicals are $21. Appointment slots are 15 minutes and are limited to 60 participants each. Make sure to complete and electronically sign all digital forms: Medical History, TISD Random Drug Testing Form, TISD Pre-Participation Forms, and UIL Forms Signature Page.
Before your appointment, download and print out the 2026-27 Tomball ISD Physical Form. This form must be filled out by you and the physician performing the physical. Please also print, complete, and sign the Houston Methodist Permit Form.
Plan accordingly and arrive 10 minutes before your appointment time check-in: Lower Lobby Parking Lot. Bring the Houston Methodist Permit Form, Physical Form, and proof of appointment/physical purchase.
After your physical is complete, turn in your complete Tomball ISD Physical Form to the Athletic Trainers.
